DIPLOMA IN PHARMACY, UNIKL RCMP
GUIDELINE ASSIGNMENT FOR SEMESTER 5 & 6 STUDENTS
This assignment will cover 15% of your total grade. Your assignment must contain minimum 5 pages (1.5 spacing/ Font 12 Times Roman) inclusive of supporting evidence i.e. prescription, medication notes etc.
Due date: Before 28th of August
Topics: -The effect of medication therapy on (name an individual) or
-Mr. Y experience of taking (name the drug/s) in treating his (name the illness) or
-An in-depth study on Pn Y medication therapy experience
Purpose:
- To learn firsthand from patient’s experience on the impact of medication therapy in modifying diseases/ lifestyle/ mobility etc.
- To relate medication therapy with patient medical condition
- To create interest/ awareness/ empathy on the suffering of the patient
- To provide a clear understanding on medication therapy in controlling long term illnesses
Getting started:
- identify an individual who is friendly and co-operative, must be on long-term medication therapy for a chronic illness
- get basic information about patient demographic, education or socio-economic background/ lifestyle/ current medical condition
- list current medications (provide a sample prescription/ medication order)/ physician/ clinic/ hospital
- interview patient for - impact of medication (the expected effects/ the side effects)administration time, drug storage, drug cost , adherence, etc
- impact of medication on lifestyle/ modifying diseases etc
- patient short term/ long term expectation
- other relevant factors
Recommendation:
- Preferably the patient is a stranger to you for greater knowledge/ experience
- Patient suffers from chronic diseases such as cancer, epilepsy, skin, psychotic condition, rheumatic disease, Alzheimer, Parkinson, drug abuser or other terminal conditions. to a lesser extent may be asthmatic, diabetic or cardiovascular patient.
- Get the most interesting case which could provide ideas or in-depth knowledge about the medication therapy
